My suggestion is that MainWP would check the ZIP file during the initial upload, before sending it up to the sites, in order to test if the archive contains a valid Wordpress plugin. Sometimes ZIP files downloaded from marketplaces such as Codecanyon contains not only the plugin file but also documentation, examples and other stuff. If one forgets to check for that beforehand, then MainWP will send the archive to the sites, uncompress them and of course that would result in the plugin not being installed and activated, and leftover files are now residing on the server.The Plesk Wordpress toolkit already implements such validation feature.
